RELIABILITY ANALYSIS USING FMEA METHOD TO IMPROVE PREVENTIVE MAINTENANCE OF HEMODIALYSIS MACHINE AT RSUD KABUPATEN GRESIK

ABSTRAK

Mesin Hemodialysa merupakan salah satu instrument medis yang penting bagi pasien penderita gagal ginjal yakni sebagai life support. Oleh karena itu analisis reliability pada mesin Hemodialysa sangat menentukan kehandalan dari komponen komponen penyusunnya karena mampu memprediksi tingkat kehandalan sebagai bentuk performansinya. Metode FMEA Failure Modes And Effect Analylsis merupakan metode kualitatif yang menganalisis RPN Risk Priority Number tertinggi dengan mengidentifikasi mode-mode dan akibat kegagalannya. Hasil dari analisis reliability mesin Hemodialysa HD Althin Tina Bexter dengan serial no.16410 secara kualitatif menunjukkan bahwa Dialyzer mempunyai Risk Prioritas Number tertinggi sebesar 120 dan secara kuantitatif diperoleh reliability total Rst 0877 pada saat t 0 bulan sehingga interval waktu T preventive maintenance yang tepat untuk sistem yakni dilakukan dalam waktu kurang dari 1 bulan. Adapun nilai availability yang dimaksud adalah At0983 dengan t 12 bulan.


ABSTRACT

Hemodialysis machine is one of important medical instrument as life support for patient who has kidney damage. Therefore the reliability analysis in Hemodialysis machine determine the reliability of its arranger components because it can predict the reliability level as Hemodialysis machine performance. The FMEA Failure Mode and Effect Analysis is qualitative method that analyze a highest RPN Risk Prioritas Number by identifying modes and the excess of its failure. The result from reliability analysis of Hemodialysis machine HD Althin Tina Bexter serial no. 16410 qualitatively show that Dialyzer have highest Risk Prioritas Number amounting to 120 and quantitatively optain generate total reliability Rst 0877 when t 0 month so the right time interval T of preventive maintenance for the system is not more than 1 month. The availability value mentioned above is At0983 with t12 months.
