Industri merupakan salah satu unsur penting dalam pembangunan, namun aktivitas industri akan diikuti dengan dampak negatif terhadap lingkungan. Dampak negatif tersebut adalah jika dihasilkan limbah cair yang sangat berpotensi merusak lingkungan. Risiko lingkungan ini muncul jika Instalasi Pengolahan Air Limbah (IPAL) tidak mampu mengolah limbah cair sehingga melebihi standard baku mutu. Oleh karena itu dibutuhkan aplikasi sistematis dalam meminimasi kemungkinan terjadinya risiko terhadap lingkungan. Dalam penelitian ini dilakukan identifikasi dan analisis risiko lingkungan berdasarkan konsep manajemen risiko lingkungan dengan menggunakan metode Failure Mode and Effect Analysis (FMEA) dan Root Cause Analysis (RCA). Di akhir penelitian diberikan usulan atau rekomendasi untuk mitigasi risiko. Berdasarkan hasil identifikasi risiko dan akar penyebab terjadinya risiko terdapat empat risiko yaitu limbah cair tumpah, penurunan kualitas efluen, bakteri WWTP mati, dan pencemaran lingkungan. Dari masing-masing risiko yang teridentifikasi ini diketahui tingkatan risikonya berdasarkan matriks risiko bahwa risiko limbah cair tumpah, penurunan kualitas efluen, dan pencemaran lingkungan termasuk low risk dan risiko bakteri WWTP mati termasuk high risk.

Industry is one of important element in development, but the industrial activity will be followed with negative impact to environment. The negative impact is if liquid waste can damage the environment. Environment risk will be happened if the Waste Water Treatment Plant (WWTP) unable to processing of liquid waste so that exceed quality standard. Therefore needed the systematic application to minimize risk probability throughout environment. In this research identified and analysis environment risk based on environmental risk management and used Failure Mode and Effect Analysis (FMEA) and Root Cause Analysis (RCA) as a method. In the end given recommendation for risk mitigation. Based on the result identification and root cause of risk there are four risk such as liquid waste spilled, degradation of quality effluent, death of bacteria, and environmental pollution. Based on the matrix risk that liquid waste spilled, degradation of quality effluent,and environmental pollution are represent low risk and death of bacteria is high risk.
