Description:

Proses konstruksi pada proyek jalan memiliki berbagai macam ketidakpastian yang pada akhirnya akan memunculkan berbagai macam risiko. Risiko adalah efek kumulasi dari peluang kejadian yang tidak pasti yang mempengaruhi sasaran dan tujuan proyek. Dampak risiko dapat mempengaruhi produktivitas, prestasi (performance), kualitas dan anggaran biaya proyek. Oleh karena itu, Tugas Akhir ini meneliti tentang identifikasi dan analisa risik, menggunakan dua metode, yaitu metode Failure Mode and Effect Analysis (FMEA) dan metode Fault Tree Analysis (FTA). Identifikasi dimulai studi literatur dan survey pendahuluan. Responden penelitian ini adalah Project Manager, Site Engineeering Manager, Site Operational Manager, dan K3LM yang menangani Proyek Pembangunan Jalan Lingkar Nagreg V Bandung. Identifikasi risiko dilakukan dengan cara mencari failure mode pada tipa proses pekerjaan. Untuk mengetahui penyebab dari failure mode tersebut digunakan metode Fault Tree Analysis (FTA) sedangkan untuk mengetahui efek dari failure mode dgunakan metode Failure Mode and Effect Analysis (FMEA). Dari FTA akan didapatkan nilai probability kejadian risikosedangkan dari FMEA akan diketahui nilai dari besarnya dampak yang ditimbulkan oleh risiko tersebut. Untuk menilai tingkat risiko dilakukan dengan cara mengalikan nilai dari tingkat Severity dan Probability untuk mencari risiko yang paling dominan. Sebagai langkah terakhir dari Tugas Akhir ini adalah menentukan tindak mitigasi yang tepat terhadap risiko yang dominan. Dari hasil penelitian ini diketahui bahwa risiko-risiko yang relevan terjadi adalah kelongsoran, keterlambatan, retak, keruntuhan, material hilang lampu tidak nyala atau mati dan data tidak sesuai dokumen kontak. Risiko yang dominan terhadap proyek adalah kelongsoran pada pekerjaan Timbunan Tanah, terjadinya kelongsoran pada pekerjaan Galian Tanah, kelongsoran pada pekerjaan Pengecoran Lereng, Terjadinya keruntuhan akibat pergeseran tanah pada pekerjaan Bronjong dan keterlambatan pada proses pekerjaan Timbunan Tanah. Sedangkan tindak mitigasi yang dapat digunakan adalah selalu mengamati prakiraan cuaca, menggunakan staf berpengalaman, memberikan alat pengaman diri (APD) pada pekerja, dan memperpendek jarak quarry tanah.

The construction process on the road project has a lot of uncertainty that leads into the risks. Risk is the cumulative effect of an uncertain chance event that affect the goals and objectives of the project. The impact of risk can affect productivity, achievement (performance), quality and budget of the project cost. The objective of this research were to indentify and analyze risks using two methods, the method of Failure Mode and Effect Analysis (FMEA) and Fault Tree Analysis method (FTA). Identification activity starts from literature study and preliminary surveys. As the respondens were the Project Manager, Site Manager Engineeering, Site Operational Manager, and a handle K3LM Ring Road Construction Project Nagreg V Bandung. Failure mode was indetified in each activities root cause of failure was analyzed using FTA and its effect was identified using FMEA. Output from FTA was probability value, while output from FMEA was effect from risks. Risk assessment was conducted by multyplying severty level and probability value to obtain the highest risk. Risk mitigation was a risk to highest risks. Relevant risks were sliding, delay, carcking, colleps, loss of material, light does not turn on or off and file does not match the contract documents. Result from the study, they are sliding in land fill activity, sliding in land cut acivity, sliding in casting slope, collapse in bonjong activity and delays in land fill activity. While mitigation that can be used is always watching the weather forecasts, use of experienced staff, providing safety devices themselves (APD) to workers, and shorten the distance of quarry land.
